(Honest - this truly was a medical instrument in earlier days!)

The tobacco enema was sold as a miracle cure by what are normally described as snake oil salesmen. The device would infuse tobacco smoke into a patient's rectum for various exaggerated medical claims, including resuscitation of drowning victims. It never worked and soon became known as a useless exercise, leading to the popular expression "he's blowing smoke up your ass".
